_The Case Management Program offers individualized support for people with chronic mental illness to reach treatment outcomes. Areas of support focus on housing, employment, education, daily activity, financial and legal issues, psychiatric and medical care, social and personal growth. The intensity of services is continually adjusted to address the full spectrum of treatment from crisis intervention to independence of case management. The service is provided to meet individual needs and incorporates natural support to integrate to the maximum level of independence.
